Hope Case Management promotes health maintenance as a way of life for people with HIV in New York City. We offer intensive case management services to individuals and families in New York City dealing with the impact of HIV/AIDS. Case management services are provided by a staff of bilingual, culturally sensitive professionals who are experienced in working with men and women with HIV/AIDS.
Hope Case Management staff works with New York City HIV population to teach them how to become effective managers of their own health and social service needs. Through service oriented HIV counseling & HIV education groups, clients are taught to identify their service needs early. Program staff will personally accompany clients to appointments as needed, and will advocate for them as required.
Hope Case Management teams coordinate clients' overall medical and psychosocial care, including helping clients enroll in substance abuse and alcohol treatment programs and coordinating medical care early enough to help clients avoid hospitalization in New York City. -
